Rolling up a rag (or even somethig as large as a dishtowel) and putting it between a person's teeth does not prevent them from screaming or speaking. The words are very garbled, but the scream comes out crystal clear.
However, it is very uncomfortable. You cannot close your mouth, drool starts rolling down your chin, and you feel like you aren't breathing properly. Also, if the person gagging you is strong enough, they have pretty much complete control over you.
I learned this from personal experience today, and decided to tell you about it, because it is interesting trivia to know those old cowboy movies kinda lie to you.
Summary: being gagged does not stop noise, but is pretty much horrible in every other way.
