Pyramid Head makes his first appearance in the Wood Side Apartments, standing behind a wall of bars, which separates him from James Sunderland, and bearing a red aura. He does not move or react to James in any way, but his presence does cause the radio to emit static.

- I got freaked out more when you actually fight him. Enemies with 1-hit-kill attacks usually freak me out like that. XD;
